Get the FREE Pfizer COVID-19 Vaccine for ages 5-11 on November 15th from 7am to 11am. Pop-Up Event location will be held at our school. Please read the family letter attached for answers to frequently asked questions.

NOTE: Students ages 5-11 must be accompanied by a parent or legal guardian, or by an adult designated by the parent/guardian, while receiving their vaccine.

If the student is accompanied by an adult who is not the parent or guardian, the vaccine provider must either obtain verbal consent from the parent/guardian by phone at the time of vaccination, or, parent/guardians may now instead choose to send a completed consent form with the adult designee.
